A simulator that answers back
For decades medical simulation has been scripted. A scenario plays out the same way whatever the learner does, and the manikin is a stage rather than a patient. The E-SIM Pro 3 was built to end that. It is the world's first complete AI-powered ECMO simulator, and its physiology responds: clamp a line, change the sweep gas, adjust pump speed or give volume, and pressures, blood gases, ECG and lactate move the way a real patient's would. We call it Responsive Simulation, and it is the reason a team leaves a session having practised judgement rather than sequence.
Everything in one platform
The Pro 3 brings together what used to require several machines and several rooms: ECMO cannulation, perfusion and circuit management, troubleshooting, ventilation, whole-body POCUS ultrasound, AI-driven haemodynamics, ECPR, and Impella, ECPELLA and IABP support. Underneath it are the Flow-Sync engine and the H-SIM artificial-intelligence haemodynamics core, which compute real physiology in real time instead of replaying a recording.
Built by someone who has stood at the bedside
The Simulator Company was founded in London in 2019 by Sagar Haval and Babita Haval, and became the first UK company to manufacture manikin-based ECLS simulators. Sagar is a clinical perfusionist and ECMO specialist with twenty-five years in the field: he designed an ECMO programme's circuit and protocols and became its first ECMO Coordinator, helped grow ECMO provision across South and Southeast Asia from fewer than five centres to more than a hundred, delivered over two thousand training sessions, and performed the United Kingdom's first Normothermic Regional Perfusion simulation.
That history is why the Pro 3 behaves the way it does. Every decision in it was argued out against a real clinical memory of what goes wrong, how quickly it goes wrong, and what a team actually needs to have rehearsed before it happens.
Simulation is about people
The machine is never the point. What matters is the clinician standing in front of it at two in the morning, and whether the hours they spent rehearsing hold when a real patient deteriorates. We are grateful to the clinicians, perfusionists, intensivists and educators who have tested our work, told us plainly where it fell short, and helped shape what it became. Shaping heroes, saving lives is not a strapline to us; it is the measure we hold the work to.
